• List of allergies
• List of medications

Before your clinic appointment

• Remember to arrive 15 minutes before your appointment
• Bring the appropriate documentation (see above) 
• Missed appointments may result in a $25 no show fee
• To cancel an appointment – Use the Deacon Health Portal or call 336.758.5218
• Please note that arriving more than 5 minutes after your scheduled appointment time may require the visit to be rescheduled

What to remember

• Please tell the front desk staff if you have Medicaid
• Visits may incur charges
• Deacon Health does not participate in or file any insurance policies. However, students are encouraged to bring their insurance card to their appointment in case an X-ray is ordered or referral is needed.
• Allergy Alert – For the comfort and safety of others, please do not wear perfume or cologne to your appointment
