Allergy Injection Clinic Information
Deacon Health provides an allergy injection clinic at WFU. In order to receive your injections here, the following documentation must be submitted along with allergy serum vials.
Medical Record Release
Please download and complete the fillable PDF form to request and/or release your medical records. Note that a completed and physically signed medical release form is required anytime a patient’s medical records are requested.
The form allows the requester to specify the information and service dates they would like released. Records can be mailed, faxed, or picked up in person at Deacon Health during business hours. This form can also be used to request medical records from other facilities to be shared with Deacon Health.
Some information, including lab results and immunization records, can be accessed from your Deacon Health Portal at any time without requiring a release. You can use your Deacon Health Portal to upload your completed form, or email it to deaconhealth@wfu.edu.
To respect our students/patients and the confidentiality of their medical records, Deacon Health is not able to accept or rely on a “blanket” release or authorization form, including a power of attorney or similar document. Please do not upload such documents to the portal.
International Travel Itinerary Form
Students who are traveling to international destinations should complete the following questionnaire to assist Deacon Health in determining if vaccines and medications are needed to prevent travel-related illnesses. Deacon Health will review this information and contact you about scheduling an appointment if vaccines and medications are needed for your particular itinerary. Travel services will stop 2 weeks before the last day of classes.
